- How long has your egg donor program been in operation?
- How do you recruit egg donors? Will you work with a donor we’ve found?
- What is your live birth rate?
- Is counseling provided for the donor? For her partner?
- What medical screenings do you perform?
- What are the average costs? What is the fee payment structure? What services do those fees cover?
- Will we be matched with a donor, or can we select the donor? On the basis of what criteria?
- Do you allow for contact with the donor?
- Will the egg retrieval take place on-site, or do you work with local physicians and hospitals?
- Are all eggs from one donor used for one recipient or are the eggs split between recipients? If they are split how are they divided? Is there a primary recipient and a secondary recipient?
- How many eggs are guaranteed to a recipient, and, if there are fewer, is the fee reduced?
- How is the recipient protected in the event a donor does not take her meds or drops out of the program?
- Do you freeze extra embryos?
- What type of legal counsel is offered to us and to the donor?
- Do you require that all parties enter into a donor agreement?
- Do you maintain medical histories of the donors or the children?
- Do you have a registry for the donor and the child to exchange information when the child reaches maturity?
